WHAT’S REALLY HAPPENING
A stranger touching the body can read as danger. In distress, fear can override reasoning - so what looks like defiance is very often fear, not choice.
THE C.A.L.M. APPROACHfour moves, in sequence
C
CONNECT
Approach from the front, at eye level. Name first, warm face, one message.
A
APPROACH SLOW
One step, one task. Low, unhurried voice. No looming, no rushing.
L
LET THEM LEAD
Offer a real choice. Place the cloth in their hand - let hands begin.
M
MOVE ON
If distress rises, stop and soothe. One or two gentle tries, then defer.
IN THE MOMENTtap a choice to follow the path

FIRST - RULE OUT A MEDICAL CAUSE

New, sudden, or intense resistance - or pain, injury, fever, or new confusion - is a red flag. Do not keep trying: stop and tell your nurse today.

WHAT TO SAYexact words that calm - and words that escalate
✓ SAY THIS
“Hi Rose, it’s Maria. I’m glad to see you.”
connect before you touch
“Let’s get you fresh and warm.”
invite - don’t announce
“Blue towel, or the white one?”
offer a real choice
“You hold the cloth - I’ll help you.”
let their hands begin
“Let’s take a break. I’ll be right back.”
retreat, never force
✕ NOT THIS
“Calm down.”
reasoning is offline right now - they can’t
“It’s time for your bath now.”
a command sounds like a threat
“Let me do it, it’s faster.”
strips away all control
“Stop fighting me.”
brands them the aggressor
“Okay sweetie, be a good girl.”
baby talk raises resistance

DISTRESS
Measured reductions - randomized controlled trial

Person-centered bathing sharply reduced aggression, agitation, and distress across randomized trials (Sloane et al., 2004; Gozalo et al., 2014) and program evaluations. Reducing elderspeak measurably lowers resistance (Williams et al., 2009, 2017).

Remove the triggers, and you become the brake the person’s brain has lost.
